Dear Parents/Guardians of Students in Grades 6, 7, & 8 for the 2023-2024 School Year,
If your child wishes to participate in any athletics/cheer, a sports physical is required in order to participate. The Athletic Office should have this on file prior to tryouts of any sport/cheer season.
All 6th-grade students, or 7th and 8th-grade students who haven't turned in an initial sports physical and wish to try out/participate in any athletics/cheer, require a sports physical. Please refer to the attached form, which you can take to your clinic to have completed.
If your child has received a sports physical since 6th grade, an Alternate Year form should be completed in order to participate. Please ensure that we have the initial sports physical on file in the Athletic Office.
These forms are also available at the JH Office/District entrance. You can also submit the sports physical form that the doctor's office provides.
